For those instances where you will need to utilize the Health Network One service request form, we ask that you use this updated form. The contract provider for all laboratory services for all CarePlus members is LabCorp. It is the only laboratory that may be used for specimens. It is imperative that your staff is aware that they can only utilize LabCorp to avoid non-par charges being incurred by your practice. If you do not have an account with LabCorp, you may contact them at LabCorp for assistance. You can always refer to the patient’s benefits to verify coverage.
