Credentialing Application and Provider Rights:
All contracted providers will be notified via phone call or e-mail and will be allowed to review any information obtained from outside sources (ie: state licensing boards, malpractice insurance carriers) to support their credentialing application. HN1 is not required to make references, recommendations or peer-related protected information available to the applicant. Providers will also have the ability to correct erroneous information that was identified through primary source verification that differs from their application and they have the right to obtain credentialing status upon request. All corrections must be submitted within 14 days and the changes will be presented to the Credentialing Committee. All status inquiries can be sent to [email protected]
